Freshmen

Requirements:

  1. Accomplished Student Application Form (download here.)
  2. One (1) piece 2×2 ID Picture, white background with nametag
  3. Certified True Copy of Grade 11 Grades (1st and 2nd Semester)
  4. Certified True Copy of Grade 12 First Semester Grades – with a GPA of at least 80%
  5. Good Moral Character Certificate
  6. PSA Birth Certificate (formerly NSO)
  7. 1 valid ID (Ex. School ID, Passport, Driver’s License

Transferee

Requirements:

  1. Accomplished Transferee Form (from the Registrar’s Office)
  2. Student Application Form (download here)
  3. TOR/Informative Copy of Grades
  4. Certificate of Transfer Credentials
  5. Good Moral Character Certificate
  6. Two (2) pieces  2×2 ID Picture, white background with nametag
  7. PSA Birth Certificate (formerly NSO)
  8. 1 valid ID (Ex. School ID, Passport, Driver’s License

Admission Procedure

S.Y. 2020-2021
  1.  APPLICATION FOR VELEZ COLLEGE ENTRANCE TEST (VCET)
    • Step 1: Sign-up for Initial Registration. Please click on the link for the form: https://forms.gle/egsqkAS1CsjtdqbT9
    • Step 2: For evaluation purposes, Email complete requirements by sending a scanned copy to velezadmission@gmail.com (2 weeks before the examination date) .All requirements will be evaluated by the Admission Committee Incomplete requirements will not be accepted. If qualified, you will receive a notification within three (3) working days
    • Step 3: Upon notification, pay the non-refundable testing fee of Php 600 at Velez College Cashier (located at 2nd floor CIM Building, beside CVGH) and ask for the receipt.
    • Step 4: Present your testing fee receipt at the Office of Student Services and Affairs (OSSA) to secure testing permit.
    • Step 5: Interview with the Dean / Admission Committee.
    • Step 6: Take the Velez College Entrance Test (VCET) as scheduled.
  2. CONFIRMATION OF SLOT FOR ENROLMENT
    1. Obtain confirmation form from the Dean’s office of the different Colleges, or you may download the form here.
    2. To reserve your slot, Submit the following to the Dean’s office
      1. Accomplished confirmation form. Make sure that the form has been signed by the parent/guardian and the student.
      2. Photocopy of parent’s/guardian’s valid ID who signed the confirmation form.
      3. VENUES:
        • BS Biology – CAS Faculty Room ASB Ground Floor
        • BS MedTech – College of Medical Technology Dean’s Office, 2nd Floor CIM Building
        • BS Nursing – College of Nursing Dean’s Office 2nd Floor CIM Building
        • BS OT/PT – College of Occupational and Physical Therapy Dean’s Office floor CIM Building
      4. Mail Confirmation (For outside Cebu):
      5. Mail in Accomplished Confirmation form. Make sure that the form has been signed by the parent/guardian and the student.
      6. Photocopy of parent’s/guardian’s valid ID who signed the confirmation form.
      7. Photocopy of payment/deposit receipt with applicant’s name to : 
        • OFFICE OF STUDENTS SERVICES AND AFFAIRS (OSSA)
        • ASB, VELEZ COLLEGE

    3. Pay the non-refundable reservation fee of five thousand pesos (PhP 5,000.00)at the Velez College Cashier or to any BDO/ CHINABANK branch using the following accounts:
      • BDO: Account Name: VELEZ COLLEGE INC.; Account #: 6 1000 716 21 
      • Chinabank: Account Name: VELEZ COLLEGE INC.; Account #: 175 030 4314 
  3. MEDICAL & PHYSICAL EXAMINATION (Schedules to be announced)
